Hypertension Recognition, Treatment and Control in Community Practice
Nearly 1 in 4 adults in the U.S. have hypertension (HTN), and the prevalence is expected to increase to ~30% by 2025. There is currently insufficient knowledge on how to deliver optimal hypertension (HTN) care in routine practice. In addition, there is limited knowledge of how HTN care varies by patient, provider, and clinic system factors in community settings. This project is designed to address these gaps in knowledge. We will evaluate HTN management over 5 years across three large health care delivery systems, and identify factors associated with HTN recognition, treatment and control. |
