Improving Contraceptive Use in High-Risk Women

  This project lays the ground work for conducting a large-scale theory driven, multi-component clinic-based intervention to improve contraceptive use and reduce unintended pregnancy rates among young, low-income, and minority women receiving care at family planning clinics and engage fellows and junior faculty in the development of patient oriented research to improve contraceptive use behavior among women at high-risk for unintended pregnancy.

Trends in Primary Cesarean Deliveries in Kaiser Permamente Northern California

Our primary aim is to examine temporal trends in primary cesarean deliveries among singleton births from 1995 to 2007 in Kaiser Permanente Northern California using information from the Kaiser Permanente Pregnancy Glucose Tolerance Registry. We will explore whether the trends in cesarean deliveries vary by maternal BMI, glucose tolerance level and race/ethnicity.

Effect of Intimate Partner Violence on Women's Health Care Utilization

This project will (1) Determine the patterns of current health care utilization (mental health, primary care, specialty care, emergency department, inpatient, pharmacy, laboratory and radiology) and costs among women who are and are not experiencing intimate partner violence (IPV); (2) test the hypothesis that  women currently experiencing IPV have lower rates of receipt of routine preventive care (PAP smear, mammogram, CRC screening) than those not experiencing IPV; and  (3) determine how clinical identification of IPV-affected women affects health care utilization.

Comparing the long-term and real world effectiveness of initial management strategies for DCIS

Use data from the DCIS study (initially funded via the Cancer Research Network) to generate real world evidence regarding risk of recurrence and treatment for recurrence after initial diagnosis and treatment.  Specifically, we will 1) Determine type of surgery and the rate of complications from surgery including reconstructive surgery after recurrence, 2) Use the variables in the study to create a personalized risk score for use in predicting the rate of recurrence in an individual patient and 3) Study the timing, side effects, compliance of tamoxifen therapy and the relationship between duration of tamoxifen on timing of local recurrence and the use of tamoxifen at diagnosis of DCIS recurrence.

A Direct Observation Study of Provider-Patient Communication About Contraception

This study intends to answer the question of what aspects, if any, of contraceptive counseling lead to better adherence and continuation of methods by women. In addition to asking if improved communication between providers and patients positively impacts contraceptive use, we are interested in answering if provider-patient communication about contraception differs by patient race/ethnicity.

CRN Pilot: Cancer Risk Factors for Young Adults

For this one-year pilot study, the two data-contributing sites we will identify eligible active members of the health plan beginning 1/1/2007 who are 20-39 years old. Each site will obtain IRB approval and Data Use Agreements, and prepare programs to meet the specific aims of the pilot study: 1) to describe the completeness of data on risk factors, confounders and effect modifiers to study the association between obesity, diabetes, metabolic syndrome and common cancers in young adults at KP Southern and Northern California, and 2) to build a prospective cohort of young adults to investigate risk factors in the young adults which will serve as a potential CRN resource for other seeking to collaborate on studies.

H1N1 Flu and Pregnancy: The Kaiser Permanente Experience

In this study we will examine all pregnancies within the four regions of  KP populations to identify women with ILI (influenza like illness) at any time during pregnancy at a time period when there was significant local circulation of swine flu in the immediate surrounding community.  We will then compare their pregnancy and infant birth outcomes to: (a) the outcomes of women who were pregnant at the same time period but did not have a medical care visit for ILI during their pregnancy, and (b) the outcomes of women who sought medical care for ILI during pregnancy during the current seasonal influenza season and the seasonal influenza of the past 5 years.
