Cardiovascular diseases remain the leading cause of death and disability in the United States and one of the fastest rising causes of death and disability in the world. Through studies such as Coronary Artery Risk Development in Young Adults (CARDIA) and such partnerships as the Cardiovascular Research Network and the American Heart Association Outcomes Research Center, our researchers study the epidemiology, prevention, management and outcomes of cardiovascular diseases.

Epidemiology: Oxidative Stress and Early Atherosclerosis

The renewal of this study utilizes and expands the infrastructure of the CARDIA year 25 examination planned for 2010-2011 to obtain additional blood and urine samples and to perform additional biochemical analyses to elucidate early stages in the pathogenesis of CVD and etiologically related conditions.

Effectiveness of medical therapy, endovascular therapy, and surgery for peripheral arterial disease

The objective is to inform current practice regarding peripheral arterial disease (PAD) treatment as follows:  1) to assemble a registry of PAD patients (n=60,000) in 2 integrated healthcare delivery systems and collect detailed chart abstracted procedural data (n=3,000) for those who undergo endovascular or surgical revascularization; 2) to describe demographics, co-morbidities, medications, and risk factor control of diagnosed PAD patients by medical therapy only, endovascular therapy or surgery; and 3) to assess the comparative effectiveness of surgery versus endovascular therapy for PAD.

Mental Health Disorders & Medication Adherence & Outcomes in Hypertension, Atrial Fibrillation & Venous Thromboembolism within the CVRN

To examine the relationship between three prevalent mental health disorders (depression, anxiety disorders, and ADHD), their associated therapies, and outcomes for hypertension, atrial fibrillation and venous thromboembolic disease within the NHLBI-sponsored Cardiovascular Research Network (CVRN). These outcomes will include intermediate markers such as medication adherence and surrogate outcomes (e.g., systolic blood pressure and percentage of time spent at a therapeutic INR), as well as long-term clinical outcomes such as thromboembolic events (e.g., acute myocardial infarction, ischemic stroke, recurrent venous thromboembolism) and major bleeding.

Improved Estimates of the Comparative Treatment Effects of CABG and PCI

Gender, Obesity, C-Reactive Protein and Oxidative Stress

Epidemiological data and a trial in smokers suggest that Vitamin C supplementation may decrease C-reactive protein, a marker of inflammation associated with heart disease. The current study is a randomized trial of vitamin C versus placebo among obese patients with elevated levels of C-reactive protein.
